Lakeshore Family Care ready to turn your children into
“CHAMP”s (Children’s Healthy Activity & Meal Program)

PROGRAM: Kids receive a monthly calendar and stickers when they sign up. Parents then make log entries by placing a sticker on the days where their child has eaten a healthy food and/or performed a healthy activity. Their calendar is exchanged for a prize at the end of each month. The program continues for 12 months, and begins and ends with a Wellchild exam.
GOALS: To promote healthy lifestyles in children by encouraging active play and healthy food choices. To encourage follow up with Well-child visits.
PARTICIPANTS: Kids ages 6 – 12 years old
LENGTH OF PROGRAM: 12 months (12 different prizes)
ACTIVITIES / FOODS THAT EARN STICKERS: 1 hour of active play and/or child must eat or try one of these healthy foods: fruits, vegetables, nuts, whole grain items (bread, rice, noodles)
HOW TO SIGN YOUR CHILD UP: Child must be a registered patient at Lakeshore Family Care. Annual Wellchild visit must be up to date, or one will be scheduled prior to beginning the program. The program will then be reviewed and all materials will be given to the parent at that time.

Lakeshore Family Care assists in local “Back to School” drive |

Lakeshore Family Care contributes to MGHP’s Mission Services in its goal to help Loaves and Fishes Food Pantry with their “Back to School” sale. Loaves and Fishes Food Pantry is a local non-profit food bank that holds an annual back to school drive to assist underprivileged children in our community.
Our employees showed their support by amassing a sizeable amount of school paraphernalia, including: pencils, pens, glue, notebooks, pads of paper, and backpacks. |
