Lakeshore Family Care - New Patients

We value new patients in our practice. We also know how difficult it can be to find a new physician with whom you are comfortable in dealing with. And so, we invite all potential new patients in for a “meet and greet” appointment. Simply ask for this as your initial encounter and, at no charge, we will arrange for you to come into the office for a short tour and a brief meeting with a provider of your choice. In addition, we have included a new patient general information form here that you can print off and fill out prior to your first appointment.
